Divorce Preparation
Client Information Form (Disso. or Mediation)
Confidential Client Information form which needs to be completed and returned to our offices prior to the initial client consultation. The information is protected by the attorney-client privilege and if completely filled out, provides for the most rewarding consultation experience.
This court form is a useful tool for clearly illustrating property division issues. You should complete it and bring it to your initial client consultation if you have any concerns involving the division of property.
This form is mandatory in dissolution matters as part of the statutory financial disclosure requirements. It is more detailed than the Property Declaration above and for the purposes of a client consultation, you may complete it and bring it instead of the Property Declaration if you wish.
This Income and Expense Declaration (FL-150) is a mandatory court form. As part of the statutory financial disclosure requirements, this form reports your actual and average monthly income in addition to reflecting all of your costs and expenses. Completing and bringing this form will provide for a more useful initial consultation as it will help the attorney address any support issues.

This form must be completed by the adopting parents to reflect any expenses they paid with regards to their adoption.
California has special guidelines for adoptions involving Native American children. This form is necessary in those circumstances.
